Something I haven't seen mentioned a lot is that they never actually say they dropped the first bomb. They only say that they will if it doesn't happen naturally. I think it's more likely that the US or China dropped the first bomb. Mainly because if Vault-tec did it then they would have made sure Janey was with her mother not Cooper. Unless thats just a bit of poor writing.
