Doc was saying to Alma that maybe going cold turkey from her drug use when so much is going on isn't great timing to try to get sober. He wants her out of Deadwood because he knows she's in danger. Al wants her high so that they can work her and get her gold claim. But now Seth will be looking after her business affairs and they are shit out of luck.

So I think this episode is 5 days from Bullock and Star leaving Montana after hanging that guy and 4 days from first meeting Bill, Jane, and Charlies on the road outside of Deadwood.
