for the caves they're on a set that was intricately made and to add some realism in a meta sense when they're crawling through the small hole you can see on the bottom of the screen where the camera was being dragged to film the shot. This is in fact the correct version; in the US the ratings board thought the original ending was too scary so they cut the movie at Sarah in the car being scared by the hallucination but she actually gets out. I know that there's an even shorter version that cuts out excessive blood and gore.
