Why is Natasha "Natasha the Goblin" when everyone else has a last name?

At birth, a goblin will receive what we would consider a first name and will simply be called "[Name] the Goblin." If two goblins share a name and you need to clarify which one, you would specify a physical feature such as "Valorie with red hair" or "taller Tiffany." Where they get a last name, which we would consider closer to what we call a title, is when they have a career and gain a level of recognition.
Right now, Natasha is only a part-time nurse at the physical therapy clinic she works at as she hasn't yet proven how good a job she can do. However, once she has proven that, gets hired on full-time with benefits, and gets recognition as an official physical therapist, she will be known as "Natasha the Healer."
Until then, she is simply "Natasha the Goblin."
