It's universal healthcare and a certain minimum is offered and even when you have to pay extra it isn't expensive but you are given the opportunity to pay more for more "luxury" level of service. The split is you usually end up paying around 20% of the cost and the government pays 80%

I always come back to this episode just for that scene! π But thanks to that, IU actually got into yoga. I also find it really cute how Hyori and Sangsoon's first thought is always, "I have to show this to Jieun." She really became like their child!! I'm excited for the next episode. Thanks for this! π
