Maybe Bumi was just the king's son? It might just be a standard monarchy, same as the Fire Nation. Either way I don't think it's ever explicitly said. Also, Bumi is just the king of Omashu, not the Earth Kingdom. Omashu is a big city within the Earth Kingdom. Kinda weird that it has its own monarch now that I think about it...
