When Frieren makes the comment about her and Fern having the same diet it's right after she noticed that Fern has gotten taller than her. Then Frieren glances at Fern's chest. Then you see the two piles of rocks that kind of resemble boobs. In other words, that was Frieren realizing that Fern has not only gotten taller but also developed bigger boobs than her. So she is kind of confused/upset as to why she developed small boobs. So as wholesome as this show is that was indeed a very subtle boob joke.
