Caleb is shown with a dagger over his head because Phillip (Belos) stabbed him with the ritual dagger that we see throughout the series. Since he's a figment of Belos's damaged mind the dagger is sort of a representation of Belos's choices that lead him here. The catalyst to everything was his choice to murder his brother.
