The necklace was never cursed in a literal sense. It's more of a superstition because she gave it to her little brother as a gift first, who then died a day later. Then years later she gave it to her lover, and then he died in the next battle. Its never been actually cursed with a jutsu or spell, its only been deemed "cursed" because both people Tsunade has given it to has died after getting it.

The kyuubi was about at lights out and then was able to increase after the tear drops. Pure speculation but it could have been her will and bloodline making the necklace work for its intended purpose. Naruto's willpower is the biggest part of it though and obv she did her healing on him.

The curse they spoke of was just because coincidentally the two people given the necklace by so not a happen to die. The healing abilities of Tsunade restored Naruto to a stable condition then she healed his hand to prevent excessive blood loss. The tears were just a visual representation of the intense emotion and determination that went into what Tsunade was doing as she decided that what happened to her loved ones would NOT happen to Naruto. And once she had healed him enough Naruto's will brought him back to consciousness long enough to tell her he won.

Still, Kabuto said he cut his heart chakra off and somehow severed his connection to the nine tails. Seemed truthful as the Fox was confused as it was going black. So imo, either Tsunade's healing worked, or Naruto's own inner strength/a combo of the two kept him alive. I do think the show likes to ask the question of what is Naruto without the fox. Kabuto even says something to that effect. And we're shown time and time again, he's got huge chakra reserves and 'the guts to never give up'. He went the whole fight without the fox and mastered (in his own way) the Rasengan. So my take is, Tsunade gave him a chance and his pure determination pulled him back. Shown by the way he's still thinking about winning the bet.
