She was part of a band of roughians, but she was unconscious on the ground when they made her into mince meat.

This... this is par for the course in a D&D campaign. "The safe word is 'Mahalo'." That phrase has stuck with me since I heard it on a sitcom from the 90s, so that's all that comes to mind.
