Remember in 4x02, Kate is looking for it to learn control. It was her reason for bringing Derek back to his teen years. And we know it wasnβt in their family for hundreds of years because Peter says thereβs a part scratched off in the back where βMade in Chinaβ used to be. So itβs nothing special, but yes it is used as a placebo. Also, remember when Peter says βDesert Wolf,β malia says it means coyote. So the desert wolf isnβt a wolf at all. (Hence malia being a werecoyote). Thatβs all I can say without giving anything that hasnβt been revealed already. Derek evolved from the normal werewolf transformation (human with wolf characteristics, like Scott, Liam, etc.) to being able to turn into an actual wolf which is to be a special ability revealed in 3x08. You can see that he had gotten stronger just from him singlehandedly taking down a berserker.
