No spoiler but Itachi indeed was sick. He had something called Chakra sickness or something like that. When he fought Sasuke we has already done for . Even without fighting Sasuke he didn't had much to live. Beside you know the blindness problem. I didn't count that in. Probably that's why he even gave Sasuke the chance to settle the score between them. That's what I know so far. Itachi was far from healthy or otherwise he would have never lose vs Sasuke.
