If I saw correctly, when Klein looks at Neil with spiritual vision, the thing he sees beneath the dark spot is, I believe, the silhouette of a woman, Neil's wife. As for the man from the cathedral's came for Klein about his sequence passage, to put it simply, certain entities who have already reached high sequences can tempt others, like the Hidden Sage for Neil, who whispered to him the method of reincarnation through alchemy.
