Luke in Japanese culture calling someone by their first name is an act of intimacy that only usually happens with close friends, family, or lovers. So yes Rias is sad because she feels that issei feels that closeness with the others but for her he’s more formal so she believes that he doesn’t feel as close to her. The anime skips this but in the novels in Issei’s inner thoughts he’s thinking how he really wants to call her by name but doesn’t feel it’s appropriate yet until he becomes stronger/higher ranked. It’s basically a typical anime misunderstanding.

The Chinese version is "Xiaomao" btw. Which is the Nickname old granny Suiren gave Maomao in Apothecary diaries. While Maomao simply meant "Cat Cat".

Kids like to play this "nipple guessing game" where they will suddenly try to poke both breasts of their female friends, see if they hit the nipple and get a reaction out of them. It's the same principle as the "Thousand years of pain" prank, where they will hold both hands together with the indexes extended and try to suddenly and strongly poke the ass of a guy with his guard down. My guess is that Azazel is looking for thought processes that could rile up Issei's carnal desires, as it seems to be his strongest source of power yet, so he may have looked into Japanese culture and decided to use that as his inspiration. He doesn't strike me as a particularly lustful guy that would go thinking about poking nipples out of nowhere for no reason, you get me?
