In addition to what @Seraphem said above, the other issue is that Sadie is ordering people (Marines and prisoners specifically) to address her by the honorific "-chan." In the Japanese language/social system, you don't really get to *choose* the honorific people call you by -- they call you by what feels appropriate and organic for their relationship with you; you don't decide that yourself. So Sadie is (a) ordering people to address her as if they have an affectionate relationship with her, and (b) saying it to prisoners (whom she oppresses and tortures) and Marines (who are essentially her professional colleagues). That's why they're all baffled. It's like if back when you worked construction, you went to your foreman and said "I DEMAND that you call me Little Boomie, you dick!"

A lot of the still frames come directly from the manga so its always pretty cool to see a highly detailed manga panel adapted for the anime. I also always like the big group shots they do in one piece. Sanji uses the honorific -swan (san) for Nami which is the more formal, respectful honorific and -chwan (chan) for Robin. you'll also see -sama used a lot for highly respected people or people with high status.

There's not really easy ways to translate the honorifics into English. But closest way to describe it is "-Chan" would mostly be used for someone that in English you'd call 'sweetie'. Someone close to you and you want to use a term of endearment for. Also used for small kids, animals, and in general things that are really cute and endearing.
