You nearly did the most cited meme "theory" here: Crocodile is Luffy's mom. Most people believe that Croodile was a female before after this scene but I doubt it. Oda shows us way too often that Ivankov can turn male into female but only reveals in a short sentence that Ivankov can change anything in a human body... And I don't know if Ivankov would really threaten someone because of a gender change in the past.

It's not that Ivan's hormone is changing them into what they want to look like, it's that Ivan is probably really good at recognizing trans people. He probably gets approached by people who want their sex changed all the time, and knows how to recognize subtle clues that indicate body dysphoria. That's my headcanon, otherwise he's just turning some people into the opposite sex whether they want it or not, which is low-key a little terrifying.
