Air Cooler Bottlenecks

With AIO's for the most part I know it's rad size/density and the fans, but with air coolers due to how there are so many different designs I was wondering what would be the cause there. Is it poor quality heat pipes, a bad layout of them all clumped together (Possibly like the Hyper 212?), not enough fin area? A mixture of all three? How much would the mounting system affect thermals so long as it's good? Could the Hyper 212's mounting system for example be poor enough to actually result in a slight thermal bottleneck due to not pressing down enough? I know the fans can be a factor as well but those can usually be replaced pretty easily.”
