It's an upgraded variant of the previous Ashley series: I changed the camera's depth of field to make it less blurry; Ellie is brighter while the room is darker (I didn't dodge and burn. I simply changed the parameters of the walls to make them reflect less light).
As for the belly, I had a quintuplet pregnancy in mind (you can nearly hear Ellie cursing her incredible fertility), and so I added a few veins. They weren't added post-render, but before (I drew them on Ellie's skin surface via GIMP). To make it appear as if the veins are, well, under the epidermis, I set the "veiny skin surface" as the base layer, while I added the "normal skin surface" as the translucency layer (which lies above the base one).
