Yes, they are conservative with the pain meds. Because of that, and because I weigh about twice what many Japanese do, I usually double or triple the doses they prescribe. Never had a problem doing that.

I'm getting depressed at watching the efficiency with which your issues are addressed. I had to wait 9 hours in the emergency room last December after absent-mindedly mixing a cold medicine with a prescribed antibiotic. Did a number on the heart rate, I tell ya.
Alright, now what I REALLY want to know is this: I hear the Japanese aren't very big on pain meds, and that they have more of a "tough it out" approach. (Heck, maybe I even heard it from you.) How is the dosage the doctor prescribed working for you?
