To add to the discussion: I think the 'hand thing' is also a reference to conductors of classic music (to which he turned with his last recordings). So in a way his left was conducting his right hand. Anyway, a nice playful touch (don't need the drug thing to explain it although you never know ;-)) and a great hymn to what it feels to call NYC your mental home wherever you might come from.
