A man not above bending rules and taking bribes.

I was gonna be a little stinker and say that we can't 100% confirm he is Dr Hibbert's brother, but he does specify that he's searching for his long lost twin brother, so unless there happens to be another identical third man, it is most likely that this is Dr Hibbert's brother. This also most likely means he is Bleeding Gums Murphy's younger brother too:

Hibbert's proclivity for losing siblings (it's only the two, but that's two more than I've lost) is never followed up upon. The Orphanage Director never appears again, tragically.

This scene is very funny, and marks a big shift into pushing how stupid Homer can be. He gets progressively dumber each season for a while, and his inability to recognise the orphanage director's attempts to help him is a strong indicator of how stupid he is becoming. It's dangerous to make your main character too stupid, and therefore unable to function, but the show plays fast and loose with Homer's cognitive abilities as the scene requires.
