This is the second in the series of alignment based shrines. This is the shrine to Beelzebub, representing Lawful Evil. Beelzebub is a hard one to pin down as an archdevil. In some places he's the lord of flies. In Pathfinder, he rules the seventh layer of Hell, the cold realm of Cocytus. In 5e, Beelzebub is known as Baalezebul and was the lord of Maladomini, the seventh layer of the Nine Hells, and one of the most powerful archdevils. Formerly a celestial archon known as Triel, he bore the titles Fallen One, Lord of the Flies, Lord of the Lies, and the Slug Archduke. The last refers to the form he was cursed with after the Reckoning of Hell; Asmodeus has made his natural form that of a filth-covered giant slug with tiny arms.
Historically, Beezlebub is a name derived from a Philistine god, formerly worshiped in Ekron, and later adopted by some Abrahamic religions as a major demon. The name Beelzebub is associated with the Canaanite god Baal.
In theological sources, predominately Christian, Beelzebub is sometimes another name for the Devil, similar to Satan. He is known in demonology as one of the seven princes of Hell. The Dictionnaire Infernal describes Beelzebub as a being capable of flying, known as the "Lord of the Flyers", or the "Lord of the Flies". (source)
In this case, his form is as the slug archduke designed by Duncan Shadow Louca who collaborated with me on this shine (Thanks Duncan!). If you have not seen it before, check his stuff out, and consider backing him on patreon.
