Okama is a queer sub-culture unique to Japan, characterized largely by effeminate, cross-dressing gay men. Its closest Western analogue would be drag queens, though it may also include under its umbrella AMAB who are non-binary or gender fluid, and potentially even trans women. Bon Clay identifies as an Okama, and depending on who's doing the subtitles, it's often changed to "queer" or "transvestite" in English subs, and it simply wasn't brought up back in Alabasta. Even the Japanese version of the anime changed his "Okama Kenpo" to "Ballet Kenpo" back then, since while many reclaim it to use as a form of self-identification, "Okama" can also be used pejoratively.
