While it certainly was introduced quite late in the story and is justifiably controversial, I wouldn't go as far as to say Kaguya returning came out of nowhere. Recall when the Ten Tails became enraged by seeing the tailed beasts inside Naruto, as if it had more of a mind than we were led to believe? More recently, the Divine Tree spoke to Madara, and then when the Infinite Tsukiyomi is cast we see Kaguya's face smirking in the sky. One other point only applies to the manga I think as in the anime they excluded this part of the scene at the time, namely that at the end of Hashirama and Madara's battle, we see Black Zetsu was observing them from afar and Hashirama senses that someone is watching but is too fatigued to find out who it is. This though showed that Madara's belief that he created Black Zetsu was false.
