A prior authorization is a requirement to be met before you start treatment. It can be for either a drug, surgical procedure or other means. Meaning you must get the insurances approval to do treatment. Typically with drugs your insurance will only cover cheap brand name drugs that have been out for a while. If it’s not approved it means they will not cover any cost and all of the cost will come out of your pocket if you choose to pay for it. Oh and a nice caveat is that does not count towards your deductible since they did not approve it
