Something that I've always believed (based on it's location) is that Ritsuko's mom died hitting Balthasar, the aspect of her as a mother. The other thing I will ALWAYS believe (even in spite of Anno saying otherwise - I'm fully employing death of the author here): Misato killed Kaji. That they gave Misato back her gun, and them telling her "the problem has been resolved" when it clearly has not been, the way she says "I see" and asks about him, and to have the immediate scene after be Kaji so familiar with who shows up, followed by Misato being clearly sad before even seeing the message left on the phone. Sorry, Anno, you can't tell me that wasn't the intended read here, even if you changed your mind afterwards.
