This video, for example, redirected me to a different page where I was asked to pay an additional 10$ per month to download it even if I connected with my Patreon. Now it seems to work fine. I dunno, maybe it was a glitch or something.

You click the link, then click the login with Patreon button on the page the link takes you to, press the button giving permission for Patreon to share your membership information with the website, and you should be able to download the video. I have a Gold membership and this is how it works for me anyway.
